AGREEMENT
I herby apply to Nature's Sunshine Products, Inc (NSP), for Membership or Distributorship authorization in the United States for one year. I understand and agree as follows:

1. APPLICATION INFORMATION. The information contained in the application is complete and true. I am at least 18 years of age. No product purchase or other payment, other than the application fee ($20 member or $40 distributor), is required to become a member/distributor. As a Member/Distributor I may buy and sell NSP products and advance to NSP's Manager ranks. I understand that this “Member/Distributor Agreement” is binding upon me as a Member/Distributor or Manager.

2. SCOPE OF MEMBER/DISTRIBUTOR AGREEMENT. This “Member/Distributor Agreement” is only one part of the full agreement I am entering into with NSP. The full agreement is found in the “Policies and Procedures” manual, published in the “NSP Member Kit” or “NSP Distributor Kit”. NSP may amend this manual from time to time. I understand that the “Policies and Procedures” as published and as may be amended and published are an inseparable part of this Member/Distributor Agreement I am signing and are fully and legally binding upon me in accord with paragraph 4 below.

3. PROVISIONS EXPLAINED IN THE “POLICIES AND PROCEDURES”. The following items, among others, are set forth in full in the “Policies and Procedures”: (a) my status as an independent contractor, (b) my lack of authority to legally bind NSP, NSP's officers, directors, and employees, and other NSP Members, Distributors and Managers, (c) my responsibility to pay government taxes, (d) NSP's responsibility to pay me in the event I am sued, (e) statements I am not allowed by law to make as a Member, Distributor or Manager, (f) termination and renewal of my Membership/Distributorship, and (g) arbitration of any controversy.

4. NO REPRESENTATIONS. Within ten days after the “NSP Member Kit” or “NSP Distributor Kit” is delivered to my mailing address, I will read the “Policies and Procedures”, published therein. Within these ten days, I may request in writing from NSP's Legal Department and explanation of or desire to negotiate any provision in the “Policies and Procedures”, or I may notify NSP in writing that I desire to reject this Agreement. I agree that declining to contact NSP within these ten days indicates that I understand, accept, and agree (a) to comply with the “Policies and Procedures” as published or as may be amended and published from time to time by NSP and (b) that the “Policies and Procedures” are made part of this “Member/Distributor Agreement” and are legally binding upon me.

5. Phone-In Agreement must be mailed or faxed within 30 days of sign-up date.

NOTICE: I may cancel this Agreement within ten days of the date the “NSP Member Kit” or “NSP Distributor Kit” is delivered to my mailing address, as explained in paragraph 4 above. A full refund for any products and materials purchased and fees paid can be obtained by contacting my sponsor or NSP.
